The Hawaii State Department of Health (DOH), Project LAUNCH seeks a qualified community-based, non-profit organization in Hawaii to:
• Facilitate an inclusive process to design a community hub model to promote young child wellness defined as a state of positive physical, emotional, social, and behavioral health.
• Develop a program for a community hub that includes convening an Advisory, conducting screening and assessments, providing training, conducting outreach, supporting family empowerment.
• Develop a workplan to oversee implementation of the model design.
• Use a Quality Improvement process to prototype a screening, training and outreach initiative.
• Develop an evaluation plan to measure the hub’s ability to effectively meet
