Description
Repair planter box spalls & delaminations as seen in PDF #39 through #67 marked in RED of the Airport Viaduct as per PDF repair location map shows. Approved HDOT fiber mesh concrete patch repair materials are to be used. An HDOT approved plybond adhesion product between original and new repairs will be used for quality cohesion, fiberglass rebar will be used replacing steel rebar that is less then 25% of its original condition. Several locations will need to have repairs performed at night for ease, safety and volume of traffic. It is highly recommended that visiting each location in order to scope extent of repairs before offering bid. Proof of experience with these types of repairs must be in good standing with proof of same or bid will be rejected

Instructions
  • RESPONSE TO THIS SOLICITATION: Only responses submitted through HIePRO shall be considered for award.
  • QUESTIONS ABOUT THIS SOLICITATION: Unless otherwise indicated, questions regarding this solicitation must be directed to the Buyer listed.
  • TRANSACTION FEES: The Awarded Vendor shall pay a Transaction Fee of 0.75% (.0075) of the award or estimated award, not to exceed $5,000 for each award. This transaction fee shall be based on the initial award amount or estimated amount, and Contractor shall be responsible for payment of the fee to Hawaii Information Consortium, LLC (HIC), the vendor administering the HIePRO. Payment must be made to Hawaii Information Consortium, LLC (HIC) within thirty (30) days of receipt of invoice.
  • OFFER DEEMED FIRM/AUTHORITY TO SUBMIT OFFER: Submission of an electronic response to the State of Hawaii constitutes and shall be deemed an offer to sell the specified goods and/or services to the State of Hawaii at the price shown in the response and under the State's Terms and Conditions. The electronic response submitter certifies that he/she is authorized to sign the response for the submitting vendor and that the response is made without connection with any person, firm, or corporation making a response for the same goods and/or services and is in all respects fair and without collusion or fraud.
  • COMPLIANCE, DOCUMENTATION AND HAWAII COMPLIANCE EXPRESS: Vendors are required to be compliant with all appropriate state and federal statutes. Proof of compliance (compliance documentation) is required. VENDOR REGISTRATION IN HAWAII COMPLIANCE EXPRESS (HCE) Proof of compliance/documentation is obtained through Hawaii Compliance Express (HCE). Vendors may register in Hawaii Compliance Express (HCE), a program separate from HIePRO. The annual subscription fee to utilize the HCE service is currently $12.00. Allow 2 weeks to obtain complete compliance status after initial registration. It is highly recommended that vendors subscribe to HCE prior to responding to a solicitation. The vendor is responsible for maintaining compliance. If the vendor does not maintain timely compliance in HCE, an offer otherwise deemed responsive and responsible may not be awarded. NOTE: State agencies may check HCE for compliance at any time. Non-compliance may result in a vendor not receiving an award, delay of payment, or cancellation of award.   • HAWAII GENERAL EXCISE TAX (GET): Unless otherwise stated in this solicitation, vendors shall include all applicable taxes in the price submitted. The Hawaii General Excise Tax (GET) shall not exceed 4.712% for the island of Oahu and 4.166% for the islands of Maui, Kauai, and Hawaii.
  • ADDITIONAL FEES AND CHARGES: Vendors shall include all applicable fees, charges, surcharges, shipping/handling, delivery, or any other charges associated with this solicitation in the price submitted.
